Maintenance documentation standards

Every DME provider offering ventilator services must maintain

  • Maintenance and service logs – maintain and provide records for all ventilators issued to Synapse Health patients
  • Technician competency – provide documentation of technician training and competency in ventilator servicing, including manufacturer certifications where applicable
  • Service contracts – if maintenance is performed by a third party or under a manufacturer service contract, submit proof of the active agreement
  • Preventative maintenance status – label every ventilator in service with a PM sticker showing the date and hours the maintenance was done and when it is next due

Maintenance and service logs must record:

  • Ventilator make, model, and serial number
  • Date of maintenance or service
  • Description of maintenance performed
  • Name or identifier of the technician who performed the service
  • Verification that the maintenance aligns with manufacturer-recommended intervals

  • Provide 24/7 emergency support for ventilator-dependent patients, covering device troubleshooting, servicing, and replacement.
  • Establish and manage the patient’s emergency preparedness plan, including notifications to local emergency services.
  • Provide loaner equipment during servicing or replacement.
