How Synapse Health clinical programs work, what DME providers in the network are expected to contribute, and the requirements every provider partner must meet.
Synapse Health clinical programs are designed to improve patient outcomes, quality of life, and the effective use of durable medical equipment. Comprehensive care goes beyond providing equipment: it takes clinical oversight, patient education, care coordination, and collaboration.
What our clinical philosophy emphasizes
- Patient-centered care – personalized support and education so each patient gets the full benefit of their equipment
- Quality and compliance – clinical and regulatory standards that protect patient safety and treatment effectiveness
- Collaboration and innovation – working with DME providers and prescribers on evidence-based practices and new technology
- Education and training – ongoing resources for patients, caregivers, and DME providers on proper equipment use and management
Your role in supporting clinical programs
DME providers deliver this in practice. We expect our provider partners to align with the clinical philosophy above and contribute through:
- Timely, accurate equipment provision. Patients receive the prescribed equipment promptly, within service level agreements, and appropriate to their clinical needs.
- Patient education and support. Clear instructions, training, and resources so patients can use their equipment effectively.
- Quality assurance and compliance. Industry standards, accreditation requirements, and company policies are met.
- Documentation and evaluation. Accurate records of patient interactions, equipment delivery, and follow-up.
- Data and outcome tracking. Participation in performance monitoring and reporting.
- Care coordination and patient safety. Proactively identify and report patient concerns, safety issues, clinical opportunities, and care gaps.
Requirements to support clinical programs
- Regulatory compliance. All federal, state, and local regulations, including Medicare and private payer guidelines.
- Accreditation. Current accreditation from a recognized body such as The Joint Commission, the Accreditation Commission for Health Care (ACHC), or the Board of Certification (BOC).
