United Healthcare Capped Rental Policy Update
Effective April 1, 2026, most capped rental equipment moves to a one-time purchase claim instead of month-to-month rental billing. Here is what your team needs to submit, and when.
What is changing
Starting April 1, 2026, UnitedHealthcare (UHC) is rolling out a new one-time purchase claim process for capped rental equipment. Once a piece of qualifying equipment has been rented for four months or more, providers must submit a single purchase claim to UHC instead of continuing month-to-month rental billing.
The goal is a simpler transaction for you and a smoother experience for the patient: fewer ongoing rental claims to track, and less disruption to the patient’s equipment during the transition. This update supports the ongoing Synapse Health network expansion, and the purchase claim itself is submitted directly between your organization and UHC.
Most capped rental equipment categories are eligible. A few clinical categories stay on the existing rental process, so check the equipment table below before you submit.
What you need to do
For each qualifying piece of equipment:
- Submit a one-time purchase claim to UHC. The date of service must be prior to the Synapse Health network expansion date.
- Calculate the claim total. Take the purchase rate from your UHC Medicare & Retirement (M&R) Agreement and subtract the rental amounts already completed.
- Submit in accordance with your contracted purchase terms.
- Obtain prior authorization if it is required for the specific equipment type.
Example calculation
| Line item | Amount |
|---|---|
| Equipment | Hospital bed |
| Purchase rate | $1,601.16 |
| Rental rate | $133.43 per month |
| Completed rental payments (4 x $133.43) | -$533.72 |
| Claim total | $1,067.44 |
Always use your contracted UHC M&R fee schedule for accurate calculations.
Contractual reference
This update aligns with the UHC Agreement, Section 2.5: “UnitedHealthcare may at any time elect to purchase equipment being rented for a Customer. In such cases, the Facility will apply all rental payments already made toward the cost of the purchase.”
Handling claim denials
If a claim’s date of service falls before the effective date, it will still be paid as long as all other requirements are met. There is no additional deadline.
Before resubmitting a denied claim
Check all three of the following:
- Modifier accuracy – confirm the correct modifier is appended for the equipment type and rental month.
- Prior authorization thresholds – verify whether the purchase price requires PA, and obtain it if needed.
- Standard denial reasons – review according to your usual internal process.
For help with a specific denial, your UHC provider relations representative is the right point of contact.
