Synapse Health evaluates supplier complex respiratory programs against the objectives, goals and activities listed below. For each one, you submit materials that outline or demonstrate how your organization accomplishes it.

Submit any policies, procedures, program descriptions, sample reports, forms or other materials that outline or demonstrate how your organization accomplishes the objectives, goals or activities related to your Complex Respiratory Program.

Equipment covered by the complex respiratory program

The Complex Respiratory Program is inclusive of the following devices:

  • Non-Invasive Ventilation (NIV)
  • Invasive Ventilation (INV)
  • High Frequency Chest Wall Oscillation Vests
  • Cough Assist
  • PAP Devices

Patient onboarding and service alignment

  • Determine appropriate onboarding protocol for respiratory home care, through appropriate screening/evaluation/onboarding checklists for complex respiratory cases.

Equipment setup, ongoing maintenance and safety

  • Set up process for both in-person and virtual equipment setups, including how setup type is determined.
  • Set up packets for Non-Invasive Ventilation (NIV), Invasive Ventilation (INV), High Frequency Chest Wall Oscillation Vests, Cough Assist Devices, PAP Devices.
  • Patient/caregiver training checklists and materials covering equipment uses, infection control and emergency procedures.
  • Set up/discharge process including home safety assessment, initial follow up plan and development of patient plan of care.
  • Resupply replenishment process including how and when supplies are delivered/provided.
  • Ongoing equipment preventive maintenance (PM) procedures and logs, including general maintenance, service and repairs.

Clinical support and monitoring

  • Process for completion of complex respiratory patient visit protocol and documentation such as visit checklists, updates to equipment settings, assessments, outcomes, interventions, care plan updates, ventilator/equipment flow sheets and logs, compliance and monitoring, including data downloads where applicable.
  • Process for maintaining license and competency verification for all Respiratory Therapists (RTs).
  • Process for tracking, reviewing and reporting incidents and adverse events associated with complex respiratory patients.
  • Provide details on system(s) used for charting and documentation management for identification of integration opportunities.

Emergency management and disaster recovery plan

  • Process for providing 24/7 emergency support for ventilator and pap dependent patients.
  • Process for establishing and documenting the emergency preparedness plan per patient, including coordination/communication with local emergency services for high-risk patients.
