Your DME Connection
Our Connect solution streamlines the DME order and delivery process so that you get what you need when you need it – without any additional headache. Here’s how it works:
Your prescriber places the order in our digital Connect portal.
The order is routed to and fulfilled by a qualified DME provider in our network.
The order is fulfilled and delivered by the selected DME provider. Synapse Health will track your delivery in real time to ensure prompt service.
Synapse Health works directly with your prescriber and health plan to ensure order accuracy and payment processing.
